Do you feel constant pain and heaviness in your legs that no diet seems to resolve? Or are you looking to improve your body contour, but fear aesthetic promises that do not match reality? In my office, the first thing I do is actively listen to your story. If you are seeking lipedema surgery, you already know that this condition goes far beyond a simple aesthetic complaint. It is a daily physical and emotional burden that affects every aspect of your life. Many women travel from all over the country and abroad to seek a definitive diagnosis and an honest, welcoming treatment plan.
The outcome of your plastic surgery does not depend solely on the operating room. Having trained at the University of São Paulo (USP), I realized early on that aesthetic success and functional recovery are intimately tied to your daily habits. That is why we treat patients by adjusting nutrition and lifestyle even before thinking about the scalpel. As a professional with extensive experience in the field of lipedema, my goal is to map out a strategy that treats you as a whole person, validating your frustrations and providing medical sincerity.
I understand the frustration of trying to buy boots that will not zip up the calf, or the exhaustion of exercising intensely without seeing proportional changes in your legs. It is time to demystify this condition. I am here to guide you through a safe, realistic, and comprehensive journey. Welcome to our space in São Paulo, specifically at the Ibirapuera unit of Instituto Lipedema Brasil, where we merge technical excellence with true human care.
What is lipedema and why does it cause so much pain?
For decades, patients with lipedema were simply told to eat less and exercise more, as if their condition were a mere lack of willpower. This is a profound medical misunderstanding. Lipedema is not a disease of the adipose tissue itself; rather, it is a connective tissue disease in which the adipose tissue is one of the main components affected. This means that the problem lies in the structural framework that supports the fat cells, leading to a cascade of physical alterations.
Because of this connective tissue dysfunction, lipedema is not just a greater accumulation of adipocytes. There is significantly more inflammation, more fibrosis, more flaccidity, more ligament laxity, and a higher risk of orthopedic complications compared to standard weight gain. The tissue becomes dense, nodular, and highly sensitive to the touch. In our clinical practice, we identify the tourniquet sign—a sharp and abrupt demarcation of fat accumulation just above the ankles or wrists. It is important to note that colloquial terms like “harem pants” or “cuff sign” do not exist in the proper medical literature; the correct terminology is the tourniquet sign.
Pain is the most common and debilitating symptom. It is present in about 86% of patients. This constant, aching heaviness worsens throughout the day and severely limits mobility and quality of life. However, there is lipedema without pain in approximately 14% of cases. The absence of pain does not rule out the diagnosis, provided there are other clinical symptoms. Patients with disproportionate fat distribution but absolutely no symptoms whatsoever do not have lipedema. A thorough clinical evaluation is mandatory to differentiate lipedema from lymphedema or simple lipohypertrophy.
How is the lipedema treatment approached before thinking about the scalpel?
When patients search for the lipedema treatment, many hope to find a quick fix or a single highly effective pill. I must be completely transparent: there is no magical treatment for lipedema. However, there are many small, consistent interventions that, when combined, can yield a very good result. This justifies the absolute necessity of follow-up with someone who profoundly understands the subject, to provide conservative treatment for lipedema that makes sense for your individual routine.
Before any surgical intervention, we focus heavily on managing the underlying inflammation. I am a plastic surgeon focused on lifestyle, which means I use the pillars of lifestyle medicine as essential tools in my surgical practice. A multidisciplinary team for lipedema treatment is vital. This team typically includes nutritionists who design anti-inflammatory dietary strategies, and endocrinologists who help manage any associated metabolic imbalances.
Physical activity is a non-negotiable pillar. Exercises must have controlled intensity and volume, continually evaluating your individual adaptation to avoid high impact that could harm hypermobile joints. While water exercises like water aerobics or swimming are excellent due to the hydrostatic pressure that helps lymphatic drainage, they are definitely not the only ones recommended. Weightlifting, pilates, cycling, yoga, and walking are excellent options that build muscle mass, improve venous return, and support the joints. Consistency is far more important than intensity.
When is lipedema surgery the right choice?
As a USP-trained plastic surgeon, I approach the indication for surgery with immense responsibility. Surgery is indicated when the conservative management—nutrition, compression garments, and exercise—has reached its limit in alleviating pain and improving mobility, or when the disproportionate fat accumulation severely impairs the patient’s quality of life and self-esteem. The surgery and treatment for lipedema must be viewed as a continuous spectrum, not an isolated event.
I practice strict alignment of realistic plastic surgery expectations. Surgery removes the diseased, fibrotic fat, drastically reducing the inflammatory burden and often eliminating the pain. However, it does not cure the genetic predisposition to the disease. If the patient abandons their healthy habits postoperatively, other areas of the body may suffer, and the overall health will decline. My commitment is to your long-term well-being, which is why we meticulously prepare you before entering the operating room.
The ideal doctor to perform this procedure is a plastic surgeon with extensive experience in lipedema, capable of navigating the delicate anatomical structures without damaging the fragile lymphatic network. We utilize specific, tissue-sparing techniques that respect your body’s physiology while achieving the best possible functional and aesthetic outcomes.
What to expect from arm and thigh liposuction in lipedema cases?
When we perform arm and thigh liposuction for lipedema, the primary goal is functional restoration: removing the heavy, painful tissue that restricts movement and causes joint strain. Secondary, but also very important, is the aesthetic improvement of the body contour. The procedure requires specialized cannulas and a meticulous technique to extract the dense, nodular fat safely.
Because lipedema involves significant connective tissue weakness, flaccidity is a major concern after the volume is removed. To address this, I utilize advanced technologies for skin retraction during the surgery. These technologies apply controlled energy to the deep tissues, stimulating collagen production and helping the skin shrink over the newly contoured areas. This approach allows for high-performance liposculpture, optimizing the aesthetic result while minimizing the need for extensive skin excisions, whenever clinically possible.
Recovery requires patience and strict adherence to the medical guidelines. You will need to wear specific compression garments, undergo specialized lymphatic drainage, and gradually return to your physical activities. The results are transformative. With the right treatment, it is often possible to live without pain and regain the joy of moving freely.
Can health insurance cover lipedema surgery?
This is one of the most frequent questions I receive during consultations. Regarding health insurance, Lipedema was officially recognized as a disease by the World Health Organization and included in the ICD-11 (International Classification of Diseases). This was a monumental victory for patient awareness and diagnosis.
However, health plans in Brazil pay for specific procedures rather than abstract disease codes. In this case, the surgical treatment is liposuction, and that specific procedure is not currently on the ANS (National Health Agency) list of mandatory coverage for lipedema. The ANS regulates the procedures to be covered by health plans. Therefore, to date, health insurances do not cover this surgery.
We discuss all the logistical and financial aspects openly during your consultation. Our clinic provides comprehensive support to help you plan your surgical journey securely, ensuring that there are no surprises and that your investment in your health is handled with the utmost respect and transparency.
Post-weight loss body contouring: Can lipedema coexist with obesity?
It is crucial to clarify that lipedema and obesity are different diseases. One does not turn into the other. However, it is extremely common for the two to coexist. The fat accumulation of lipedema and the associated symptomatology, such as pain and heaviness in the legs, can lead to orthopedic deformities and a more sedentary lifestyle. These factors naturally aggravate overall weight gain, creating a challenging cycle.
While the metabolic risk of pure lipedema tends to be lower than in isolated obesity, when obesity is associated, the patient assumes the metabolic risks of obesity. This is where our comprehensive health approach is most vital. We help you manage your overall body composition through nutrition and exercise.
For patients who have successfully lost significant weight, either through clinical methods or bariatric surgery, we offer post-weight loss body contouring. This often involves reconstructive breast and abdomen surgery to remove excess skin and restore functional anatomy. Interestingly, even after massive weight loss, the lipedema fat on the legs and arms often remains completely resistant, highlighting the need for targeted surgical intervention in those specific areas.
It is also important to dispel a common myth: lipedema is most commonly associated with obesity, but thin women can indeed have lipedema and are often extremely symptomatic. Weight gain is not a necessary condition to make the diagnosis. Therefore, lipedema is not an exclusive condition of obese women. It affects women of all sizes.

Frequently Asked Questions (FAQ) about Lipedema Surgery
Can thin women have lipedema?
Yes. While it is frequently seen coexisting with obesity, thin women can absolutely develop lipedema. The disproportionate accumulation of nodular, painful fat in the extremities is the hallmark of the condition, regardless of the patient’s overall body mass index (BMI). Thin women with lipedema are often extremely symptomatic and require the same level of comprehensive care.
Do men get lipedema?
Lipedema in men is extremely rare. This condition is overwhelmingly predominant in biological females, usually triggered or aggravated by periods of hormonal fluctuation such as puberty, pregnancy, or menopause. When lipedema does appear in men, it is generally associated with complex hormonal problems or other underlying systemic pathologies.
Is it true that physical activity is mandatory even if it hurts?
Movement is essential, but it should never cause excruciating pain or joint damage. Physical exercises must have controlled intensity and volume. We emphasize evaluating your individual adaptation and avoiding high-impact activities. Consistent, low-impact exercises improve venous return, support the joints, and help manage the systemic inflammation associated with the connective tissue dysfunction.
