Do you feel constant pain and heaviness in your legs that no restrictive diet or intense physical activity seems to resolve? Or perhaps you are looking to improve your body contour after a massive weight loss, but you fear empty aesthetic promises that do not match reality? In my office, the first thing I do is actively listen to your story. I know the frustration of trying to buy boots that will not zip up the calf. I understand the emotional exhaustion of feeling that your own body is fighting against you, despite your best efforts to maintain a healthy routine. It is no coincidence that brazil is the global capital for lipedema treatment and advanced body contouring. Women from all over the world travel here seeking an accurate diagnosis, realistic medical care, and a comprehensive treatment plan that respects their overall health.

The outcome of your plastic surgery does not depend solely on the operating room. Having completed my medical training and residency at the University of São Paulo (USP), I realized early on that aesthetic success, tissue recovery, and surgical safety are intimately tied to your daily habits. As a plastic surgeon, I utilize tools and foundations of lifestyle medicine in my clinical practice. This means we treat patients with lipedema and those with aesthetic concerns by adjusting nutrition and habits even before thinking about the scalpel. My goal is to map out a safe journey where plastic surgery is the definitive step in a broader context of self-care.

We need to move away from the standard of selling perfect bodies. A successful plastic surgery is one that improves your quality of life, relieves your pain, and restores your mobility with absolute safety. I discuss the pros, cons, and limitations of every procedure in an extremely sincere manner. If you are looking for a magical cure or an unrealistic transformation, my approach might not be for you. However, if you value technical excellence, transparency, and a global medical perspective that treats you as a whole person, you are in the right place.

Why do patients worldwide travel to Brazil for lipedema treatment?

The international recognition of Brazilian plastic surgery is well-established, but when it comes to specific pathologies like lipedema, our country has taken an extraordinary leap in research and clinical protocols. Patients from North America, Europe, and neighboring South American countries frequently seek out surgeons in Brazil because we offer a unique combination of surgical refinement and comprehensive, humanized care. We do not look at a patient as just an aesthetic complaint; we understand the functional and emotional implications of the disease.

I have been deeply involved in this evolution. In 2015, alongside Dr. Fábio Kamamoto, I was one of the first doctors to perform this surgery for the pathology in the country. At that time, information was scarce, and many women suffered in silence, misdiagnosed with common obesity. Since then, we have developed robust protocols that focus on the patient’s comprehensive health. Today, as the Clinical Director of the Ibirapuera unit of Instituto Lipedema Brasil, I am proud to lead a facility designed specifically to welcome these patients with the highest standard of care.

Our Ibirapuera unit in São Paulo was strategically opened to facilitate access for patients coming from all over the country and abroad. Located close to the Congonhas airport, the clinic offers a welcoming and sophisticated environment where patients undergo a complete evaluation. We have structured a global multidisciplinary team comprising nutritionists, endocrinologists, and surgical staff. This structure guarantees that every aspect of your health is optimized before you ever enter a surgical center.

Furthermore, telemedicine has revolutionized our ability to provide global care. You do not need to live in São Paulo to start your treatment. Through online consultations, we can conduct thorough pre-operative assessments, align expectations, and begin the conservative management of your condition. By the time you travel to our clinic for your in-person evaluation and subsequent surgery, your body will already be in a much healthier, less inflamed state, significantly improving your surgical outcomes and recovery process.

What is lipedema and why is it not just regular fat?

Lipedema is widely misunderstood, both by the general public and, unfortunately, by many healthcare professionals. It is frequently confused with common weight gain, leading to years of misguided advice to simply eat less and exercise more. However, it is crucial to understand that lipedema is not a disease of the adipose tissue; it is a connective tissue disease, in which the adipose tissue is one of the main components affected. This structural difference changes everything about how we approach the diagnosis and treatment.

Because the connective tissue is compromised, lipedema is not just a greater accumulation of adipocytes. There is more inflammation, more fibrosis, more flaccidity, more ligament laxity, and a significantly higher risk of orthopedic complications. The architectural support of the skin and fat layers is defective, leading to the formation of painful nodules and a constant feeling of heaviness. The structural weakness also explains why the affected areas bruise so easily and feel exceptionally tender to the touch.

When diagnosing the condition, we evaluate the clinical history and physical signs. Pain is the most common symptom, present in approximately 86% of patients. Therefore, there is lipedema without pain in about 14% of cases. The absence of pain does not rule out lipedema. However, there must be symptoms; patients with disproportionate fat distribution but absolutely no symptoms do not have lipedema. We also look for specific physical markers, such as what we clinically call the tourniquet sign, where the abnormal fat accumulation stops abruptly at the ankles or wrists, creating a distinct physical boundary.

A common misconception is the relationship between this pathology and obesity. The metabolic risk of pure lipedema tends to be lower than in isolated obesity. However, lipedema and obesity are different diseases. One does not turn into the other. Yet, it is extremely common for the two conditions to coexist. The fat accumulation of lipedema and the associated symptomatology, such as severe pain and heaviness in the legs, can lead to orthopedic deformities and a more sedentary lifestyle. These facts can aggravate weight gain and trigger a cycle of obesity. When there is associated obesity, the metabolic risk becomes that of obesity, demanding immediate clinical intervention.

Understanding these nuances is the first step toward reclaiming your quality of life. The disproportionate fat of lipedema is highly resistant to caloric restriction, which explains the deep emotional distress many patients face. Acknowledging that you are fighting a physiological connective tissue disorder, rather than a lack of willpower, is often the most validating moment in our initial consultation.

How does lifestyle affect lipedema and plastic surgery outcomes?

The foundation of any successful aesthetic or reconstructive procedure lies in the patient’s baseline health. As I often emphasize, I use foundations of lifestyle medicine in my practice as a plastic surgeon to optimize your body’s environment. There is no magical treatment for lipedema, and there is no single highly effective treatment. Instead, there are many small interventions that, together, can yield a very good result. This justifies the absolute importance of follow-up with a medical team that understands the subject, to provide options that make sense for each individual patient.

Nutrition plays a critical role in managing systemic inflammation. An anti-inflammatory nutritional approach helps reduce the swelling, pain, and baseline inflammation associated with the connective tissue disorder. Our multidisciplinary team works closely with each patient to identify dietary triggers and establish a sustainable nutritional plan. This is not about highly restrictive, punishing diets; it is about nourishing the body to create the best possible conditions for tissue repair and long-term symptom management.

Physical exercises are fundamental, but they must be approached strategically. For lipedema patients, physical exercises must have controlled intensity and volume, evaluate individual adaptation, and generally avoid high impact. Movement improves lymphatic drainage, builds muscle pump action to assist venous return, and supports joint stability. Water exercises are excellent because the hydrostatic pressure naturally assists with lymphatic flow while protecting the joints, but they are definitely not the only ones recommended. Weightlifting, pilates, cycling, yoga, and walking are all excellent options depending on your specific physical condition and preferences.

The goal of these lifestyle interventions is twofold. For patients managing the condition conservatively, these habits can drastically reduce pain and prevent disease progression. For patients preparing for body contouring or lipedema surgery, optimizing nutrition and physical activity reduces surgical risks, minimizes post-operative complications, and significantly enhances the aesthetic outcome. A body that is well-nourished and active responds much better to surgical trauma and heals with superior tissue quality.

Can surgery cure lipedema completely?

When conservative measures are optimized but the patient still suffers from disproportionate volume, pain, and mobility issues, surgical intervention becomes a powerful tool. However, it is essential to establish realistic plastic surgery expectations. I tell my patients frankly: surgery does not cure lipedema completely because it is a systemic connective tissue disease. Nevertheless, with the right treatment and surgical approach, it is often possible to live without pain and regain a harmonious body contour.

The primary surgical treatment for lipedema is specialized liposuction. Unlike standard aesthetic liposuction, which focuses purely on shaping, this procedure aims to carefully remove the diseased, fibrotic fat while preserving the lymphatic vessels. We meticulously address areas such as the arms, legs, thighs, and knees, focusing on volume reduction, pain relief, and functional restoration. Because patients with this condition often have inherent tissue flaccidity and ligament laxity, simply removing the fat is not enough.

During the procedure, I use advanced technologies for skin retraction. These technologies help the skin adhere better to the new body contour, minimizing the severe flaccidity that could otherwise follow a large-volume fat removal. The specific techniques and tools are carefully selected based on the patient’s skin elasticity and the amount of volume being removed. This high-performance liposculpture approach ensures that we achieve both functional relief and an aesthetically pleasing result.

A frequent and important question involves health insurance coverage. Regarding health insurance, lipedema was recognized as a disease by the ICD-11 (International Classification of Diseases). However, health plans pay for procedures, which in this case is liposuction. Currently, liposuction for this purpose is not on the ANS (National Health Agency in Brazil) list, which regulates the procedures to be covered by health plans. Therefore, to date, health insurances in Brazil do not cover this surgery. We navigate this reality with total transparency, providing clear financial planning for our patients.

What is post-weight loss body contouring and who needs it?

Beyond treating lipedema, a large portion of my private practice is dedicated to post-massive weight loss patients. People who have gone through intense weight loss processes, whether through bariatric surgery or significant lifestyle changes, often face a new physical challenge: severe skin laxity. The loss of volume leaves behind excess skin that can cause discomfort, chafing, and emotional distress, masking the incredible health achievement of losing the weight.

For these patients, reconstructive surgeries are essential to adjust the body contour and restore functional harmony. The skin has lost its elastic capacity and cannot shrink back to fit the new, smaller frame. Depending on the individual’s needs, we perform procedures such as a tummy tuck (abdominoplasty) to remove excess abdominal skin and tighten the abdominal wall muscles, as well as lifting procedures for the arms and thighs.

Breast surgeries are also highly sought after in this context. Significant weight loss often leads to a loss of breast volume and severe ptosis (sagging). Reconstructive breast surgery aims to reshape, lift, and potentially restore volume to the breasts, helping the patient feel comfortable and confident in their renewed body. Furthermore, areas like the flanks, double chin, and back can be refined using specialized liposuction and tissue-tightening techniques to complete the global transformation.

My approach to post-weight loss body contouring is rooted in humanization and technical excellence. These patients have already undergone a profound physical and emotional journey. They require a surgeon who practices active listening and who can provide a safe, structured plan. We discuss the placement of scars, the necessary recovery time, and the realistic limits of the results. The goal is to celebrate their health victory by providing a body contour that allows them to move freely and live fully, without unrealistic aesthetic pressures.

How to prepare for a body contouring or lipedema surgery?

The journey to the operating room requires meticulous preparation. When you schedule a consultation at our Ibirapuera unit or via telemedicine, we initiate a comprehensive pre-operative protocol. This involves deep blood work analysis, cardiovascular clearance, and a detailed assessment of your current lifestyle habits. We strongly encourage pre-operative nutritional optimization to ensure your protein levels are adequate for wound healing and your systemic inflammation is minimized.

Our global multidisciplinary team will guide you through every step. The endocrinological evaluation is vital to ensure that there are no underlying metabolic or hormonal imbalances that could compromise your recovery. The nutritional plan will focus on anti-inflammatory foods and proper hydration. Physical therapy and lymphatic drainage may also be recommended before surgery to improve tissue quality and educate you on the post-operative recovery exercises.

Mental preparation is equally important. Surgery is a significant physical trauma, and the recovery process demands patience and resilience. I ensure that expectations are strictly aligned. We discuss the swelling, the bruising, the initial discomfort, and the timeline for seeing the final results. By being completely honest about the challenging aspects of the recovery, I empower my patients to face the process with confidence and peace of mind, knowing exactly what to expect and knowing that our team is available to support them continuously.

Frequently Asked Questions (FAQ)

Do thin women get lipedema?
Yes. Lipedema is most commonly associated with obesity, but thin women can indeed have the condition and are often extremely symptomatic. Weight gain is not a necessary condition to make the diagnosis. Therefore, lipedema is not an exclusive condition of obese women. The disproportionate fat accumulation can occur independently of overall body weight.

Can men have lipedema?
Lipedema in men is extremely rare. This condition is predominantly female. When it does occur in men, it is generally associated with hormonal problems or other underlying pathologies. The vast majority of clinical data and surgical protocols are structured around female physiology due to the disease’s demographic presentation.

Does lipedema turn into obesity?
Lipedema and obesity are different diseases. One does not turn into the other. However, it is extremely common for the two to coexist. The fat accumulation of lipedema and the associated symptomatology (pain, heaviness in the legs) can lead to deformities and a more sedentary lifestyle, facts that can aggravate weight gain. Recognizing both conditions is essential for comprehensive treatment.

What kind of doctor is ideal for lipedema surgery?
The ideal doctor for lipedema surgery is a plastic surgeon with extensive experience in lipedema. A plastic surgeon has the specific surgical training required to manage tissue remodeling, perform safe and effective liposuction, address skin flaccidity, and manage the complex body contouring aspects while preserving the patient’s structural health.

Do thin women have a more pronounced disproportion?
It is not true that thin women automatically have a more pronounced disproportion regarding lipedema. The visual disproportion varies greatly among individuals depending on their body type, the stage of the disease, and genetic factors. The focus should always be on symptoms, pain management, and functional impairment, rather than visual aesthetics alone.
